• Looking for the records on an existing onsite wastewater system.
• A non-refundable $50 fee is required and records may or may not be available depending on the age of the system and whether documentation was filed properly at the time of installation.

Holding tank application form (Fillable PDF)

Use this form when:

• You have consulted preliminarily with your local Environmental Health Officer regarding the suitability of holding tank for your property.
• Apply for a permit for a holding tank.

Holding tank record of installation (Fillable PDF)

Use this form when:

• The installation of a holding tank is complete. The applicant must submit this form with supporting documentation to the local Health Protection Office. A holding tank must not be used until the Record of Installation has been accepted by the Health Authority.
• A non-refundable $400 fee is required at time of application.

• You are an onsite wastewater practitioner or a professional.
• You are have completed installation of an onsite wastewater system that has been filed previously and now certifying it.

Use this form when:

• You are a registered onsite wastewater practitioner or a professional.
• You are filing a sewerage system that you have designed.

Temporary food permit guideline (PDF)

Temporary food permit application (PDF)

Use these forms when:

• Operating a temporary food facility for less than 14 days a year.
• Special events when food will be served to the public such as fundraisers and fairs.
• A caterer or a permitted food facility operator catering an event where food preparation will be done outside of your own facility.
• Please note that a $50 processing fee will be applied to this application if it is received less than 14 days prior to date of event.

Exempt foods

The following foods are exempt and do not require a permit. If you are only serving these foods, no application is required.

• Cotton candy, hard candy
• Donuts/bannock (no dairy, meat fillings or toppings)
• Whole fresh fruit or vegetables
• Coffee (black with individual creamers)
• Lemonade, ice tea, shaved ice, hot chocolate (commercial mixes)
• Muffins, baked goods (commercial source, no dairy filling)
• Pancakes, waffles (no whipped cream topping)
• Popcorn
• Popsicles, novelty ice creams
• Pre-packaged shelf stable foods (commercial source, sold in package)
