Do you know someone who is taking the initiative to improve health in his or her community? If you do, tell us their story so we can share it with the north!

Community Health StarsThe Community Health Stars program highlights these exceptional individuals and the impact they have in their communities. One Community Health Star will be selected each month from the list of nominees, and will win their choice of prize from Northern Health in addition to being highlighted in their local communities and on the Northern Health Matters blog. An ideal nominee is anyone who is helping to improve the overall health of his or her community.

For a valid nomination, the nominee's work must align with the Northern Health position papers.

I understand that the information is collected for use by Northern Health to select community health stars. Should your nominee be successful, the information you provide about the nominee (excluding contact information) will be disclosed to the public at large through various communication mediums, including but not limited to local communities and Northern Health Matters blog.

Individuals can submit nominations for monthly community health stars for the purpose of winning prizes, as well as be highlighted in local community and Northern Health Matters blog (community health star individual who is taking initiative to improve overall health in their community). Information collected will be shared as per the Freedom of Information and Protection of Privacy Act (FOIPPA) for the purpose of this program.
