Reservations for the NH Connections travel service do not require information on the nature of the procedure a patient is seeking. An Appointment Verification Form only confirms that a patient attended an appointment with a physician or other health care professional.
Any patient who decides not to undergo a booked procedure after seeing a health professional will still be able to make their return trip back home via NH Connections.
