Reservations for the NH Connections travel service do not require information on the nature of the procedure a patient is seeking. An Appointment Verification Form (sample below) only confirms that a patient attended an appointment with a physician or other health care professional.

 

Any patient who decides not to undergo a booked procedure after seeing a health professional will still be able to make their return trip back home via NH Connections.


Appointment Verification Form

Patients will need to complete an appointment verification form to use NH Connections. This form ensures that patients are using NH Connections to access health care services, and not for other reasons.


Patients will receive this form when boarding the NH Connections bus.
After you have filled out this form, the health care provider at your destination will need to sign and stamp it.


Any patients deciding not to undergo a booked procedure after meeting with an out-of-town specialist or health professional will still be able to make their return trip back home via NH Connections.
